Referrals
We provide a referral form for the convenience of the patient and the referring physician. This form lists the medical tests that are helpful to have for our first visit with the patient.
We request that the patients bring their scans images (MRI, CT, angiogram, myelogram, plain films, etc.) with them to their appointment.
Angiogram
An angiogram -- also called heart catheterization, cardiac catheterization or cardiac angiography -- is a common test used to evaluate your heart muscles, valves and coronary arteries.
To learn more about angiograms click here.
Myelogram
A myelogram is a test done for back pain. This test can help caregivers find out if other problems are causing your back pain, such as broken bones or cancer. Usually this test is not done until other treatments have been tried and have not helped your back pain.

In addition, we have included two of the forms that patients need to complete. You may print off these forms and fill them out before coming to your first appointment.

Dedicated neurosurgery suites
Two neurosurgical suites on the second floor of United Hospital are dedicated to neurosurgical procedures.
Intraoperative MRI
Intraoperative magnetic resonance imaging (MRI) is a procedure that can help improve a neurosurgeon’s ability to remove brain lesions by providing real-time images during the operation. United Hospital is installing the Neuro II 1.5 Tesla intraoperative MRI system from IMRIS (Innovative Magnetic Resonance Imaging Systems Incorporated).
The mobility of the system allows the neurosurgeon to update images quickly and efficiently so that surgical adjustments and decisions can be made with pinpoint accuracy. In addition, the neurosurgeon can use the intraoperative MRI to confirm that the entire tumor was removed before closing, thus reducing the need for additional operations.
